Testing Challenges from Requirements to Production: Real-World Lessons from QA Experience

Software testing is often described as a discrete phase in the software development lifecycle. In practice, however, testing is a continuous activity that begins at the requirement stage and extends well into production. While frameworks such as Agile and Scrum promote collaboration and early quality ownership, real-world projects rarely operate under ideal conditions.
Throughout my experience working on multiple projects with varying levels of process maturity, I encountered recurring testing challenges. From unclear requirements, inconsistent QA involvement, missing UI designs, limited test planning time, to inadequate documentation. These challenges directly affected product quality, team efficiency, and production stability.
This blog captures those experiences and examines testing challenges across the entire delivery pipeline. It covers requirements gathering to production release, supported by real examples, quality metrics, and a QA maturity perspective.
1. Requirements Phase: The Root Of Most Testing Challenges
Unclear And Incomplete Requirements
One of the most common challenges across nearly every project was the lack of clear and complete requirements. Even in Agile Scrum environments where QA participated in sprint ceremonies, user stories often lacked detailed acceptance criteria or clearly defined business rules.
A Real-World Example:
In one Scrum project, a user story requested an “enhancement to the search functionality.”
However, it did not specify:
- Which filters were impacted
- Expected behavior for invalid inputs
- Performance or usability expectations
QA had to reach out to the Product Owner multiple times to clarify expectations. Test case preparation was delayed, and assumptions had to be made to proceed within sprint timelines.
This lack of clarity resulted in multiple iterations of development and testing, increasing rework and reducing sprint predictability.
Over-Reliance On Verbal Communication
In some projects, especially those without mature Agile practices, requirements were communicated mostly through verbal discussions.
Example:
In a project without structured Scrum, QA received only a release document listing bug fixes and new features. Most explanations were provided verbally by developers or quality managers. There were no updated requirement documents, user stories, or acceptance criteria.
When defects were raised, disagreements arose due to different interpretations of what was “expected.” Without documented requirements, it was difficult to validate behavior objectively.
Impact:
- No single source of truth
- Increased dependency on individuals
- Higher risk of missed scenarios
2. Qa Involvement In Agile Ceremonies: Inconsistent But Critical
Partial Involvement In Sprint Activities
In some projects, QA was included in sprint planning but excluded from backlog grooming or refinement sessions. This limited visibility into upcoming work and reduced time for test planning.
Additionally, when QA could not attend meetings, recordings were rarely shared. Important scope discussions and last-minute changes were missed, forcing QA to react late in the sprint.
Result:
- Reduced test coverage
- Increased end-of-sprint pressure
- Reactive rather than proactive testing
In other cases, QA involvement began only after development was complete.
Example:
In a Kanban-based project, developers deployed changes continuously, and QA was informed only when a build was ready. There were no planning or review meetings involving QA.
Testing occurred without a full context, increasing the risk of missing edge cases and integration issues.
3. Development phase challenges
Incorrect Or Partial Implementation
Even when requirements were discussed, incorrect or incomplete implementations were common.
Example:
A requirement specified adding validation at both the UI and API levels. Developers implemented validation only on the UI. During testing, QA identified that API requests could bypass the rule entirely.
This defect was discovered late in the sprint, requiring code changes, re-testing, and release delays.

Missing or Late UI Mocks
UI mockups were often missing or shared late, especially in fast-paced Agile projects.
Example:

Timely access to UI designs would have significantly reduced rework.
4. Projects Without Proper Agile Or Scrum Practices
Some projects lacked structured Agile or Scrum methodologies altogether.
Testing Based On Release Notes
In such projects, QA relied on build or release documents listing changes. There were no user stories, acceptance criteria, or traceability.
QA had to:
- Reverse-engineer requirements
- Ask developers for explanations
- Compare behavior with previous releases
This approach was inefficient and increased the risk of missing business-critical scenarios.
Qa As The Coordination Hub
When requirements were unclear, QA often became the central point of communication between developers, product owners, and quality managers. While collaboration is positive, this dependency highlighted deeper process gaps.
5. Time Constraints And Lack Of Test Planning
Even in projects with good Agile practices, time for test planning and analysis was frequently compromised.
Example:
Development consumed most of the sprint, leaving QA with limited time for planning and exploratory testing. Test cases focused mainly on happy paths, increasing the likelihood of defects escaping to production.
6. Production Issues And Defect Leakage
The challenges across requirements, development, and testing stages often resulted in production issues.
Common root causes included:
- Unclear or undocumented requirements
- Late changes not fully tested
- Limited QA involvement in planning
Many production defects could be traced back to assumptions made early in the lifecycle.
7. Quality Metrics That Revealed Hidden Problems
Defect Leakage
Defect leakage measures how many defects escape to production.
Formula:
Defect Leakage (%) = (Production Defects / Total Defects) × 100
Example:
In a project with unclear requirements and minimal QA involvement:
- Total defects: ~180
- Production defects: ~25
This resulted in a defect leakage of approximately 14%, leading to customer escalations and emergency fixes. High defect leakage consistently correlated with poor requirement clarity and late QA involvement.
Test Coverage
In several projects, test coverage appeared high but was misleading due to weak requirement traceability.
When testing was based on release notes or assumptions:
- Happy paths were well tested
- Edge cases and negative scenarios were missed
True coverage was low despite high execution numbers.
Rework Cost
Rework cost included:
- Developer re-implementation
- QA re-testing
- Delayed releases
In projects where QA was involved early, rework was significantly lower. In contrast, unclear requirements and late testing resulted in multiple rework cycles, reducing overall team velocity.
8. QA Maturity Model: Observations From Real Projects
Based on my experience, QA practices across projects generally aligned with different maturity levels.
Level 1: Reactive QA
- No formal testing process
- Testing starts after development
- High defect leakage
Level 2: Isolated QA
- Test cases exist
- Limited collaboration
- QA involved late
Level 3: Collaborative QA
- QA participates in sprint ceremonies
- Acceptance criteria defined
- Lower defect leakage
Level 4: Proactive, risk-based QA
- Early QA involvement
- Focus on defect prevention
- Metrics used for improvement
Level 5: Quality ownership culture
- Quality shared across the team
- QA acts as quality strategist
- Continuous feedback from production
Most challenges I encountered occurred in Level 1–2 environments. As teams matured, many testing issues naturally diminished.

My experience has shown that investing in early QA involvement, clear documentation, and collaborative practices significantly improves product quality and release confidence.
